    A periodic inspection was conducted this date.

    5:30 AM – Upon arrival of the landfill, the inspector observed no litter outside the entrance of the landfill. One Republic Services’ front loader trash truck, one Republic Services’ roll-off trash truck and four Allied Waste’s semi transfer trucks were queued for entrance into the landfill. While driving to the active face, a SLI staff was observed filling up a water truck at the water J stand. Signs clearly marked the location of the active face.

    Tyler Richmond, SLI, joined the inspection. Lights powered by external generators were used to illuminate the active face. SLI staff was observed removing the alternative daily cover (ADC) tarps from the active face. The SLI staff on the ground was wearing a hardhat and a safety vest. A water truck was observed spraying water to control dust emission on the access road to the landfill.

    06:03 AM - Trash trucks arrived at the face. The inspector observed ADC tarps laid out on the north side of the active face. One tarp had a tear greater than 6 inches. Please repair or replace the tarp as soon as possible. The lower access road on the east side was partial inaccessible (uneven). Please assure that access roads allow for safe egress and ingress of traffic at the active face. 27 CCR 20540 – Roads. The road defect was corrected later in the morning.

    7:20 AM – On the way to the scale house, the inspector observed a leachate sump tank with a confined space warning sign. The leachate sump had a solar powered over-flow alarm. At the administrative office, the following records were reviewed: Vehicle count/Tonnage, Special Occurrence Log, and Load Check.

    Vikki Denslow (County of San Diego Department of Environmental Health (DEH)) and Jamie Harris & Antonia Gunner (SLI), joined the inspection. The Hazardous Materials Business Plan was reviewed by DEHS as the Certified Unified Program Agencies (CUPA).

    08:00 AM - The inspector observed the traffic pattern at the landfill entrance. No impacts to traffic was observed at the intersection of Mast Boulevard and West Hills Parkway. Inspector observed posted signs that indicate the landfill rules and list wastes which are not accepted for disposal. Inspector observed a rat bait station at the scale house.

    08:45 AM – Southwest of the maintenance yard, the inspector observed the area that had a subsurface oxidation event which was listed in the previous focus inspection dated 9/06/2018. Mr. Harris stated the area was monitored and there was no reoccurrence. No issues were noted.

    At the maintenance yard, the inspector observed landfill gases warning signs on the storage lockers. A fire extinguisher was observed attached to a HAZMAT Storage locker. The inspector observed a portable restroom that was recently serviced on 09/10/2018.

    09:15 AM - One Caterpillar D9 dozer, two Caterpillar D7E (electric) dozer and one compactors were used to managed waste at the active face. The SLI spotter was observed using hand signal and radio communication to direct trucks into the designated commercial tipping area. Northwest of the commercial tipping deck, SLI staff was observed shooting grade with a laser device.

    At the public hand-unload area, SLI staff was observed removing consumer electronics waste (CEW) and checking customers for unacceptable waste. Mr. Harris stated that unacceptable wastes that are removed from the waste piles are documented and properly disposed.

    Exit interview conducted with Jamie Harris and Tyler Richmond.

    At 11:37, Inspector observed DEHS and District Attorney Office preparing for a waste audit of a retail facility at the landfill.

    11:48 AM - Inspector left the site with no additional issues noted on the way out.

    On 08/30/2018, SLI provided the following Republic Service’s documents upon request by the LEA:
    1. Otay / Sycamore Special Waste Acceptance Criteria
    2. Special Waste Profile form
    3. Instruction for the Completion of Special Waste Profile
    4. Special Waste Express Profile form
    5. Instruction for the Completion of the Express Waste Profile
    These documents were reviewed for the violation listed in the previous LEA Periodic Inspection on 8/21/2018. SLI has cease accepting all wastes from the tire processing / recycling facility.

    On 9/05/2018, SLI notified the LEA that DEH issued a Notice of Violation for non-compliance to California’s hazardous waste laws and SLI also provided a copy of the reply sent to the DEH (see attachments).
