    If you are a reporting entity in RDRS, then please provide your entity information:


    Allegation
    So that the Department may investigate appropriately, describe the non-compliance and the surrounding circumstances in as much detail as possible, including all relevant facts of which you are aware. You must include the identity of the allegedly non-compliant reporting entity. Additional relevant information may include, for example:
    • What specific laws or regulations did the reporting entity violate?
    • How did the reporting entity violate those laws or regulations?
    • How and when did you become aware of the non-compliance?
    • Are other entities involved in the non-compliance, and what roles do/did they play?
    • When did the non-compliance first take place? Is it ongoing, and what is/was the frequency?
    • Do you believe any supervisors, managers, or executives of the reporting entity are aware of the non-compliance? If so, state all facts on which you base that belief and describe any actions that the supervisors, managers, or executives have taken to address the non-compliance.
    • Who else do you believe may have witnessed the non-compliance or otherwise have knowledge of relevant facts? What facts do you believe such persons have knowledge of, and when did they learn such facts? Include each such person’s name, title, email, phone number, relationship to the complaint filer, and the information that they will be able to confirm.
    In addition, provide a list of available evidence and facts upon which you base your allegation of non-compliance. Describe the evidence and how it substantiates your allegations.
